2/28/2024 0 Comments Cello repertoire graded![]() Part of the problem was that our teachers were no better, and in hindsight seemed a little too focused on who had the most advanced students whose students had played the most challenging repertoire and whose students had the most achievements behind them. This was typical behaviour for young people of our age, and perhaps it was a valuable introduction to the fiercely competitive and political nature of the world we were planning to enter. We all played in the regional and national youth orchestras together, and the annual seating auditions were a tense, unpleasant affair. There were several cellists in my age group at the music school I attended, and the competition between us was stiff to say the least. But I certainly didn’t stop taking on repertoire that required much better chops than I had in those days. I never touched that concerto again except in a few sessions in my practice room many years later, when I would set down a pile of my favourite pieces next to my chair and systematically read through bits and pieces of them. I eventually abandoned the Saint-Saëns Concerto: by the time I got to the third movement even I had to admit defeat. My tempi were all over the place and bore no resemblance whatsoever to the score directions dictated instead by my technical inadequacies, which were numerous and getting worse rather than better with my hours of hacking away at music beyond my reach rather than working on my weaknesses. ![]() ![]() I can honestly say I put a brand new spin on it, and not in a good way! I used eye-wateringly bad fingering patterns throughout – especially in the double-stop passage. I wore him down and he let me take on the Saint-Saëns. 1 the Lalo Concerto both Haydn Concertos and more. I had only been playing the cello for around five years, but had already managed to perform Bruch’s Kol Nidrei (rather messily) at the local Youth Concerto Festival, and was regularly taking repertoire far beyond my ability to my lessons and nagging my teacher to let me play it: the Saint-Saëns Concerto No. In my mid-teens I was fiercely ambitious and deeply dissatisfied at not yet having passed my grade 8 cello exam with distinction. But then I thought about myself at that age – around fifteen. I was about to write an admonishing response, telling him how ridiculous he was to be so obsessed with racing toward a relatively meaningless qualification when he should be focusing on how to become the best he could at expressing himself on his instrument. My eye was caught by a thread entitled something like this: “What is the shortest time you can take to get to grade 8?” At first I was annoyed at the silliness of such a question. I was browsing a forum for classical music students the other day.

2/28/2024 0 Comments Cms triple aimPopulation health management calls for providers to keep a patient population as healthy as possible, minimizing the need for expensive emergency department visits, hospitalizations, tests, and procedures. ![]() The Triple Aim is a framework for optimizing health system performance by focusing on 1) the health of a population, 2) the experience of care for individuals within that population, and 3) the per capita cost of providing that care. Today, the organization remains committed to improving healthcare. Founded in 1980, the IHI initially focused on healthcare best practices and reducing defects and errors in specific areas, such as the emergency department or ICU. The Triple Aim was introduced in 2007 by the Institute for Healthcare Improvement. This is part of an ongoing series designed to help readers understand the implications of reform.Īs healthcare continues to move away from fee-for-service medicine in favor of outcomes-based medicine, providers will shift their goals to align with a new initiative – the Triple Aim of Healthcare. Supplier success in a post‐reform healthcare market depends on many factors, including a fundamental and thorough understanding of the foundation of healthcare reform. But it’s worth noting that a 2018 peer-reviewed journal article reported that 21% of all 30-day hospital readmissions were due to medications, and of those, nearly 70% of them were preventable. These new Part C measures, coupled with the Part D side, mean that medication reviews and accuracy have never been more important to your Star Ratings.Pay-for-performance programs are intended to improve population health and the patient experience, while reducing cost Use telehealth resources whenever possible to ensure members aren’t waiting on a provider in your busy network.įinally, because these are Part C measures, it’s likely that many MA-PD plans will overlook the importance of the medication side of readmissions.Use every available channel to engage discharged members member abrasion takes a back seat to getting them seen.Leverage ADT, HIE, and EHR data to ensure timely outreach to members. That’s not seven days to make contact with the member, that’s seven days to get them seen. The Follow Up for Emergency Department for High-Risk Chronic Members measure has a seven-day window. MA plans that have fallen back at the start of the Annual Enrollment Period should brace for member attrition since consumers use Star Ratings as one element in their decision making when choosing a plan.įirst of all, real-time data has never been more important. While 8% may not seem like a lot, it translates to an estimated 2.4 million members no longer being in a 4-Star Rated plan (even with 74% of members remaining in a 4-Star Rated plan). Taken together, these changes sent strong signals to the industry that CMS was intent on putting teeth into 2024 Stars-and it shows in the Star Ratings performance revealed on Friday.Ĥ3% of plans earned 4 Stars or higher for 2024, down from 51% in 2023. ![]() Weeks later, CMS released its guidance reducing CAHPS from 4-weight measures to 2-weight measures. And at the NCQA Health Innovation Summit last fall, Peggy O’Kane encouraged Mickey Tripathi at DHS to bring back greater emphasis on clinical measures and outcomes. CMS’s announcement of the forthcoming Health Equity Index sent the message that those gaps need to be closed for all members, not just those who are most engaged. CMS has been signaling since the end of the pandemic that the days of loopholes and gimmickry were over and that plans would need to deliver on the Triple Aim if they expect Medicare Advantage to remain profitable.įrom the stage at RISE National in Colorado Springs, Inspector General Christi Grimm set the expectation that MA plans would need to do more than just code gaps-they’ll need to close them. In many ways, the changes outlined in Friday’s announcement came with little surprise. As strongly suspected, the impact of Tukey has put to rest any doubt that CMS is requiring new and greater accountability and ownership for Medicare Advantage (MA) plans to do more for their members. After much anticipation, CMS released the 2024 Medicare Star Ratings on Friday, October 13. 2/28/2024 0 Comments Exterior plaster wall repair![]() But sometime in the first half of the 19th century, builders began installing wooden plaster “grounds” around windows and doors and at the base of the wall. Obtaining a plumb, level wall, while working against built-up moldings, must have been difficult. ![]() Generally the woodwork was prime-painted before plastering. In early America, the windows, doors, and all other trim were installed before the plaster was applied to the wall. Not only did the basic plastering material change, but the method of application changed also. ![]() Gypsum and lime plasters were used in combination for the base and finish coats during the early part of the 20th century gypsum was eventually favored because it set more rapidly and, initially, had a harder finish. Manufacturing changes at the end of the 19th century made it possible to use gypsum as a plastering material. The lime came from ground-and-heated limestone or oyster shells the aggregate from sand and the fiber from cattle or hog hair. Lime plaster was made from four ingredients: lime, aggregate, fiber, and water. Until the end of the 19th century, plasterers used lime plaster. Plasterers in North America have relied on two materials to create their handiwork–lime and gypsum. Old plaster walls, like this one painted a faded red in a Greek Revival mansion in upstate New York, have a textural, handmade quality that modern materials can’t match. While minor repairs can be undertaken by building owners, most repairs will require the assistance of a plasterer. Plastering is a skilled craft, requiring years of training and special tools. To repair plaster, a building owner must often enlist the help of a plasterer. This system is generally better suited to historic preservation projects than dry wall. Veneer systems include a coat or coats of wet plaster–although thinly applied–which can, to a greater extent, simulate traditional hand-troweled or textured finish coats. Thus, a veneer plaster system is discussed rather than dry wall. Finally, this Brief describes a replacement option when historic plaster cannot be repaired. Dry wall repairs are not included here, but have been written about extensively in other contexts. The approaches described in this Brief stress repairs using wet plaster, and traditional materials and techniques that will best assist the preservation of historic plaster walls and ceilings–and their appearance. For both reasons, plaster walls and ceilings contribute to the historic character of the interior and should be left in place and repaired if at all possible. Plaster can also serve as a plain surface for irreplaceable decorative finishes. Of more concern to preservationists, however, original lime and gypsum plaster is part of the building’s historic fabric–its smooth troweled or textured surfaces and subtle contours evoke the presence of America’s earlier craftsmen. A building owner needs to think carefully about the condition of the plaster that remains plaster is often not as badly damaged as it first appears. It resists fire and reduces sound transmission. First, three-coat plaster is unmatched in strength and durability. But there are practical and historical reasons for saving it. Historic plaster may first appear so fraught with problems that its total removal seems the only alternative. This variety and the adaptability of the material to nearly any building size, shape, or configuration meant that plaster was the wall surface chosen for nearly all buildings until the 1930s or 40s. Plaster could be treated in any number of ways: it could receive stenciling, decorative painting, wallpaper, or whitewash. It provided a durable surface that was easy to clean and that could be applied to flat or curved walls and ceilings. Technical Preservation Services (TPS), Heritage Preservation Services Division, National Park Service prepares standards, guidelines, and other educational materials on responsible historic preservation treatments to a broad public.Ī versatile material, plaster could be applied over brick, stone, half-timber, or frame construction. 2/28/2024 0 Comments Ice box cookies recipeSometimes, I’ll enclose a small log of unbaked cookies in a little decorative bag and add a few of the baked cookies just to whet the recipient’s appetite! Icebox Cookies If you wish to let the host/hostess bake the cookies when they so desire, simply wrap a chilled or frozen cookie log decoratively and include the baking instructions. Who doesn’t love to get a bag with homemade cookies inside! They’re also fun to package in cute little bags. A festive tin filled with these delectable cookies will be a sure hit with anyone. These make great host/hostess gifts, too, either baked or unbaked. They’re also great to have on hand if unexpected guests drop by – you can pop a pan of the cookies in the oven and have your house smell tantalizing while your guests await fresh-from-the-oven cookies! Simply let the dough sit at room temperature for approximately 30 minutes or so before slicing and baking. Store the unbaked cookie logs in sealed bags in the freezer until they are needed. One of the great things about icebox cookies is that they can be made ahead and frozen, either before or after baking. These cookie logs can be made two to three days in advance of baking and kept in the refrigerator so they are a great make-ahead cookie dough. When ready to bake the cookies, simply unwrap the chilled logs and, with a sharp flat-bladed knife, cut the logs into 1/4″ slices, place on parchment-lined baking sheets, and bake for 8-10 minutes. Make sure to rotate the cylinders every hour or so during the 3-4 hours of dough chilling as this will help to distribute the weight of the dough and aid in keeping the cookie log round. Slit up the length of the cylinders, these make great “sleeves” for the cookie logs and help to keep the shape of the logs round, or reasonably so. Plan ahead and save three empty paper towel cardboard cylinders. This is because the dough needs time to firm up enough to be able to cut clean-edge slices. The dough gets divided into thirds and then each third is formed into a round log shape, wrapped tightly in plastic wrap or waxed paper and refrigerated for at least 3-4 hours before slicing and baking. ![]() ![]() Icebox cookies are very easy to make and don’t take any unusual or difficult-to-find ingredients. Use finely chopped fruit and the small currants for these delicate cookies. The richness of the butter and sugar combined with the slight chewiness of the mixed glazed fruit and currants results in a tasty and flavorful cookie. There are many varieties of cookies that beg to be made, shared, and eaten over the holiday period and these are always a perennial favorite.
